    • For First-Year Applicants:

    1. First-Year Application
      Includes all parts of the first-year application, including the Counselor Statement, the Teacher Recommendation form, and the North Carolina Residency Short Form, all of which are available separately below.

    For International Applicants:

    1. International Application
      For first-year students who are neither citizens nor permanent residents of the United States. International transfer students should fill out the regular transfer application below.


    2. Financial Certificate Form
      For students who are neither citizens nor permanent residents of the United States. This form is required to certify sufficient financial resources to pay for your education.

    For Transfer Applicants:

    1. Transfer Application
      For candidates applying for transfer admission from another accredited college or university. This application is for all undergraduate UNC-Chapel Hill degree programs except junior nursing and second-degree nursing.


    2. Transfer Nursing Application
      For junior or second-degree admission to the UNC-Chapel Hill School of Nursing.
    3. Community Standards Form
      For admitted transfer students only. Hard copy of community standards form which must be completed by an official at the last college attended and submitted to complete enrollment.

    For Educators (Counselors and Teachers):

    1. Counselor Statement
      For first-year applicants only. Serves as a cover sheet for the applicant's official school transcript. To be completed by the applicant and the school counselor.
    2. Counselor Statement Supplement (Microsoft Word)
      For school counselors only. Optional editable form that allows counselors to provide general high school information on one form that can be photocopied.
    3. Counselor Recommendation for Project Uplift
      Nomination for Project Uplift 2009 has now ended. Visit Counselor Connection to learn more about the program and check back in January 2010 for your next opportunity to nominate students for Project Uplift.
    4. Teacher Recommendation
      For first-year applicants only. To be completed by the applicant and the teacher providing the recommendation.

    Residency Forms:

    1. North Carolina Residency - Short Form
    2. North Carolina Residency - Supplemental Form
    3. North Carolina Residency - Long Form

    For Current and Former Students:

    1. Course Approval Form
      For currently enrolled Carolina students who wish to take courses at another accredited college or university.
    2. Readmission Application
      For students who were once enrolled at Carolina but withdrew or failed to complete a semester.

    Other:

    1. Part-time Classroom Studies Application
      For part-time local adult students who seek to enroll in no more than eight credit hours each semester and don't already have a bachelor's degree.
    2. Employee Undergraduate Degree-Completion Application
      Application for part-time undergraduate degree-completion program for UNC-Chapel Hill employees.
    3. Online Application Confirmation Page
      For part-time classroom studies and readmission students who have completed the online application.
