We encourage you to apply online through your UNC homepage. If you would like to submit a paper application, please use the forms below.

The downloadable forms require Acrobat®, which is available free of charge from Adobe. Please make sure Acrobat® is installed on your computer before you try to download and print these forms.

For First-Year Applicants:

• First-Year Application
  Includes all parts of the first-year application, including the Counselor Statement, the Teacher Recommendation form, and the North Carolina Residency Short Form, all of which are available separately below.

 

For International First-Year Applicants:

• International Application
  For students who are neither citizens nor permanent residents of the United States. Students should also complete the Financial Certificate Form, which is available below.
• Financial Certificate Form
  For students who are neither citizens nor permanent residents of the United States. The form for the 2009-2010 year will be available by mid-September.
  

 

For Transfer Applicants:

• Transfer Application
  The 2009 application for transfer admission will be available by mid-September. For candidates applying for transfer admission from another accredited college or university. It is for all UNC-Chapel Hill degree programs except junior nursing and second-degree nursing.
• Transfer Nursing Online Application
  You may apply online for junior or second-degree admission to the UNC-Chapel Hill School of Nursing.
• Community Standards Form
  For admitted transfer students only. Hard copy of community standards form which must be completed by an official at the last college attended and submitted to complete enrollment.
• Carolina Student Transfer Excellence Program (C-STEP) Interest Form
  Hard-copy interest form for students interested in finding out more about the Carolina Transfer Excellence Program.

 

For Educators (Counselors and Teachers):

• Counselor Statement
  For first-year applicants only. Serves as a cover sheet for the applicant's official school transcript. To be completed by the applicant and the school counselor.
• Counselor Statement Supplement (Microsoft Word)
  For school counselors only. Optional editable form that allows counselors to provide general high school information on one form that can be photocopied.
• Counselor Recommendation for Project Uplift 2008
  Nomination form for Project Uplift 2008. For counselors only. You may also nominate students for Project Uplift through the Counselor Connection website.
• Teacher Recommendation
  For first-year applicants only. To be completed by the applicant and the teacher providing the recommendation.

 

Residency Forms:

• North Carolina Residency - Short Form
• North Carolina Residency - Supplemental Form
• North Carolina Residency - Long Form

 

For Current and Former Students:

• Course Approval Form
  For currently enrolled Carolina students who wish to take courses at another accredited college or university.
• Readmission Application
  For students who were once enrolled at Carolina but withdrew or failed to complete a semester.

 

Other:

• Part-time Classroom Studies (formerly Continuing Studies) Application
  For part-time local adult students who seek to enroll in no more than eight credit hours each semester and don't already have a bachelor's degree.
• Employee Undergraduate Degree-Completion Application
  Application for pilot part-time undergraduate degree-completion program for UNC-Chapel Hill employees.
• Online Application Confirmation Page
  For part-time classroom studies (formerly continuing studies) and readmission students who have completed the online application.
  
• Release Form
  For students who are giving UNC-Chapel Hill permission to conduct a background check.
